Four years in, Escape the Backrooms is celebrating its anniversary the only way it knows how: by giving players somewhere new to get hopelessly lost.
The co-op horror hit has just added Level 421, aka Plastic Mariana, a free new level available now on PC, PS5, Xbox Series X|S, and Nintendo Switch 2.
This time, players will venture into an abandoned M.E.G. outpost linked to a children’s hospital. Expect dark corridors, patrolling Scratchers and plenty of reasons to keep moving. Then things get even stranger: no-clip into a seemingly endless ballpit and explore what appears to be the domain of a terrifying shark entity.

Along the way, you’ll solve puzzles, shoot some hoops and desperately try to find a way into Overgrowth. Simple enough — assuming you survive.
The free update marks four years since Escape the Backrooms first entered Early Access in 2022. Since arriving on PS5 and Xbox Series X|S in May 2026, the game has surpassed five million console players and landed among the PlayStation Store’s top ten downloaded games in both June and July.

Not bad for a place you’re supposed to escape from.
Level 421: Plastic Mariana is available now for free, with Escape the Backrooms also included in Xbox Game Pass Ultimate, Game Pass Premium and PC Game Pass.
Just remember: survival isn’t guaranteed.
